Boy Scouts of America require that Part A (Informed Consent, Release Agreement, and Authorization) and Part B (General Information/Health History) be completed annually for every boy participating in many of BSA and Pack 56 activities such as campouts, hikes, and other events and activities.  When parents and other family members participate in these events, Parts A & B are required for each participant.  For some activities, Part C (Pre-Participation Physical) may also be required.  Notification will be given when Part C is required.

Additionally, Pack 56 request completion of the Photograph Release and Electronic Policy forms annually.  The Event Permission-Medical Release Form may be required for some activities.  Den leaders will provide notification when this form is required.

The forms are designed to be completed electronically and printed; however, the forms can be completed by hand if desired.

Please complete Parts A & B, Photograph Release, and Electronic Policy forms and submit to your son's den leader as soon as possible.
