All Scouts must have a completed application on file. Please submit to the troop with paid registration fees.

Please fill out parts A and B.  Part C must be filled out by a physician (must be complete for extended camping trips, summer camps).  If your Scout has had a recent physical, most doctors will fill out the form without an appointment.
